何辉明： 我局于2023年3月6日作出《深圳市认定工伤决定书》（深人社建工认决字〔2023〕040000036号）。因无法联系到您，且依据民事诉讼法规的其他送达方式无法送达，现公告送达如下： 职工何辉明（男，身份证号码：511321198210******）于2022年7月28日在城建大厦施工总承包工程因日常工作受伤，根据2022年7月28日初诊记录及其他诊断结论（或病历本）诊断为左足第4跖骨近端骨折，受伤部位是左足。 该员工上述情形符合《广东省工伤保险条例》第九条第（一）项，属于工伤认定范围，现予以认定（或视同）为工伤。 如对本认定工伤决定不服的，可自本决定书送达之日起六十日内依法向深圳市人民政府申请行政复议；或自本决定书送达之日起六个月内依法向深圳市盐田区人民法院提起行政诉讼。 本决定自公告之日起经过三十日，视为送达。 特此公告。深圳市人力资源和社会保障局2023年3月29日
